There are a number of convenient, secure, and confidential ways to apply to NAFG's surrogacy program.

1. Online Submission

3. Supporting Materials

Your application is not complete without a copy of a photo ID and current digital photos. Current digital photos of you and your family, as described on the application, should be sent to inquiry@assistedfertility.com; copies of photo IDs may be sent by e-mail, fax, or mail to the address above.

We are happy to accept scanned applications and other documents, as well as digital photos sent to inquiry@assistedfertility.com.

Whether submitted electronically, by fax, or by mail, all applications are confidential.
